Abstract

Background/Context: Veterans are increasingly diagnosed with post-traumatic stress disorder and depression, and access to mental health (MH) care at the Department of Veterans Affairs is challenging. Problem/Purpose: Access to care is influenced by care coordination procedures and if those are inconsistent, patients remain in settings that do not match their health care needs. The care coordination project (CCP) was implemented to support stable patients transitioning their care from specialty-MH to Primary Care. Results: The CCP established a standardized process that facilitated care transitions from one setting to another and improved the patient’s care experience. Impact: Consistent care coordination practices can effectively achieve organizational goals, promote positive patient outcomes, and improve access to care.
