Now what?We're here to lead the way.The Alzheimer’s Association Care Facilitators of Idaho will provide personalized care consultations to dementia patients and their loved ones to support and guide them through the difficult post-diagnosis journey. Our vision is an Idaho where dementia patients and their loved ones never feel alone. In partnership with the Alzheimer’s Association, our team aims to provide one-on-one consultation services to dementia patients and their loved ones to discover the best options for legal support, financial advising, emotional support, caregiving options, and educational resources.
