Abstract
The Statewide Health Innovation Plan (SHIP) was a broad statewide initiative designed to transform healthcare delivery in Idaho through a triple aim approach to improve health outcomes, improve quality and patient experience of care, and reduce healthcare costs for all Idahoans. Through the collaboration between stakeholders and providers, Idaho successfully transformed primary health care clinics into patient centered medical homes. Patient and provider feedback received by the State Evaluation Team demonstrates improved health outcomes, perceived quality of care, successes and accomplishments made during the transformation process in Idaho.
