Predictors of Sepsis Mortality: Evaluating the Relationship Between Age, Demographics, and Bundle Compliance

Abstract
Sepsis remains one of the leading causes of mortality globally. While clinical guidelines emphasize the benefits of compliance with the 3 hour-bundle, a Surviving Sepsis Campaign guideline recommendation suggests that obtaining a blood culture before antibiotic administration, measuring lactate, administering broad-spectrum antibiotics, and providing 30mL/kg of crystalloid fluid for hypotension or high lactate (>4) may improve outcomes. However, timely recognition and delivery of fluids and antibiotics which is crucial for improving survival, remains challenging, especially in vulnerable populations. This study conducted a retrospective analysis of 3,542 de-identified sepsis cases from St. Luke's Health System to examine the relationship between bundle compliance and mortality outcome across vulnerable populations. Logistic regression was used to identify association between mortality and sex, high lactate (>4), ICU admission, AKI/ESRD, source of acquisition, and ED sepsis alert. Initial models show significant associations between rapid bundle completion and mortality were driven by confounding variables. After adjusting for confounders, shorter bundle compliance demonstrated greater protective qualities. Findings suggest that standardized recognition protocols are the primary drivers of mortality. Clinical severity must be accounted for to accurately evaluate bundle timing impact.
