Clinical Implications of Renal Disease on Sepsis Mortality and Interventional Care

Abstract
Characterized by physiological and biochemical abnormalities, sepsis presents as a potentially fatal clinical syndrome that precipitates serious impairment of vital bodily systems–culminating in widespread inflammation, tissue injury, and multi-organ failure. Furthermore, clinical literature establishes a strong association between higher mortality and pre-existing conditions. This pattern is evident in a sample of 3,620 adult and geriatric patients within the St. Luke’s Health System, where one-third of the population present with concurrent renal complications. Given these clinical demographics, this analytical inquiry seeks to address whether pre-existing renal status functions as an independent driver of sepsis mortality or serves as a proximal indicator of a more severe initial clinical presentation. Here, multivariable logistic regression modeling was used to investigate the independent predictors of sepsis mortality. By identifying that treatment effects remained constant across all populations and did not diverge based on renal status, this research illustrates how impaired homeostatic fluid regulation and septic shock more impact patient outcomes.
